Media Consent Release Form
I, THE UNDERSIGNED, DO HEREBY GRANT to the Mexican American Catholic College (MACC), my permission to
- Record my image, voice and likeness through video and/or photos
- Reproduce my verbal and written comments
I understand that these materials will be used for educational and promotional purposes, provided in response to or in connection with any evaluation, survey or interview, including being used in our college’s publications, online educational instruction videos, websites, advertising, fundraising, and marketing/communication materials.
Additionally, I understand that these materials can also be used for current and/or future use in any form of media and promotion, whether the use of such materials is for public relations, recruitment, development, or any other legitimate purpose of the Mexican American Catholic College. I understand that I will not be compensated for my participation and/or for the use of the materials mentioned above, and I will have no rights to the photographs or video taken.
I understand that if I do not wish to be photographed or recorded at an event, I must notify the photographers/videographers AND remove myself from photos/videos being taken. I understand that MACC has no control over the use of photographs or video taken by other participants or the media that may be covering an event.
I understand and agree to these terms. If I am under the age of 18, I understand my parent or legal guardian must sign this form with me.

Video and Photography – Sharing Media Content Consent Form
I give my consent to the Mexican American Catholic College (MACC) to agree to not share publicly in person or through any class materials – including through social media, and will not own or claim any rights to share any of the live and/or pre-recorded links, downloads or media files for classroom lectures without permission from the instructor, the Online Distance Learning Department and Fr. Juan Molina, president of MACC.
I understand and agree that the links and videos provided through my instructor or generated on Populi, MACCs learning management system, will only be shared within the constructs of class with my instructor and classmates only.
I understand that I will not be paid for my participation and/or use of the media and I will have no rights to the media taken. I understand if I am in violation of these terms, that a meeting with my instructor, the Online Distance Learning Department and Fr. Juan Molina, president of MACC, will take place to discuss appropriate action.
I understand and agree to these terms. If I am under the age of 18, I understand my parent or legal guardian must sign this form with me.

Formulario De Consentimiento, Autorización Y Liberación Para El Uso De Medios De Comunicación
Yo, EL ABAJO FIRMANTE, POR LA PRESENTE OTORGA al Mexican American Catholic College (MACC), mi permiso para
- Grabar mi imagen, voz y semejanza a través de video y/o fotos
- Reproducir mis comentarios verbales y escritos
Entiendo que estos materiales se utilizarán con fines educativos y promocionales, proporcionados en respuesta o en relación con cualquier evaluación, encuesta o entrevista, incluyendo el uso en las publicaciones de nuestro colegio, videos de instrucción educativa en línea, sitios web, publicidad, recaudación de fondos y márketing /materiales de comunicación.
Además, entiendo que estos materiales también se pueden usar para uso actual y/o futuro en cualquier forma de medios y promoción, ya sea que el uso de dichos materiales sea para relaciones públicas, reclutamiento, desarrollo o cualquier otro propósito legítimo de MACC. Entiendo que no seré compensado por mi participación y/o por el uso de los materiales mencionados anteriormente, y no tendré derechos sobre las fotografías o videos tomados.
Entiendo que si no deseo ser fotografiado o grabado en este evento, debo notificar a los fotógrafos/camarógrafos y quitarme de las fotos/videos que se están tomando. Entiendo que MACC no tiene control sobre el uso de fotografías o videos tomados por otros participantes o los medios que puedan estar cubriendo un evento.
Entiendo y acepto estos términos. Si soy menor de 18 años, entiendo que mi padre o tutor legal debe firmar este formulario conmigo.

Vídeo Y Fotografía: Formulario De Consentimiento Para Compartir Contenido Multimedia
Doy mi consentimiento al Mexican American Catholic College (MACC) para aceptar no compartir públicamente en persona o a través de ningún material de clase, incluso a través de las redes sociales, y no poseeré ni reclamaré ningún derecho para compartir ninguno de los materiales en vivo y/o previos enlaces grabados, descargas o archivos multimedia para conferencias en el salón de clases sin el permiso del instructor, el Departamento de Aprendizaje a Distancia en Línea y el Padre Juan Molina, Presidente de MACC.
Entiendo y acepto que los enlaces y videos proporcionados a través de mi instructor o generados en Populi, el sistema para el manejo de aprendizaje de MACC, solo se compartirán dentro de las clases, solo con mi instructor y compañeros de clase.
Entiendo que no se me pagará por mi participación y/o uso de los medios y no tendré derechos sobre los medios tomados. Entiendo que si estoy en violación de estos términos, una reunión con mi instructor, el Departamento de Aprendizaje a Distancia en Línea y el Padre Juan Molina, Presidente de MACC, tendrá lugar para discutir la acción apropiada.
Entiendo y acepto estos términos. Si soy menor de 18 años, entiendo que mi padre o guardián legal debe firmar este formulario conmigo.
